First Visit Forms

We would like to welcome you!  When you arrive for your first appointment, we require forms to be filled out pertaining to your personal, medical and dental information.  This information will enable our team to safely provide dental treatment for you. Rest assured that all information will remain confidential.

1. Click on the 2 links: Personal Document & Medical Document to download the PDF forms.
2. Print the 2 documents.
   
3. Complete both documents for each patient that is booked.
4. Bring the completed forms with you to your first visit.

Appointment modifications may be made by telephone only. Our office maintains a reserved appointment policy whereby we require 48 business hours notice should it be necessary for you to reschedule your appointment. This policy assists us in providing better service to you and all our valued patients. Please allow us to provide you with the best service possible by respecting this policy and informing us if you will be unable to attend your reserved appointment.
